General Duties

Perform intensive case management functions for youths and their families to help them cope with multiple risk factors and areas of needs. Develop and implement in a logical, timely and effective manner individualized youth development strategies to ensure that prescribed support services are provided to defined high risk students and their families. Develop and maintain mutually respectful long-term relationships with families. Identify resources and establish a referral system within and among community agencies designed to support youth and families. Identify and work with families’ social supports, including faith-based organizations, neighborhood block associations, tenant groups, community centers and clubs. Conduct regular case management meetings with parents and key personnel from school, law enforcement, lead agency and CASA to discuss progress toward goals and to modify service plans as needed and follow-up for review. Prepare and submit monthly and quarterly statistics. Conduct community outreach by planning and executing social and educational events and outings. Job Requirements

Education: 
Bachelors degree in Social Work, Counseling, Psychology or other human services field from an accredited institution.

Experience:
1-3 years experience with 6 months-1 year in substance abuse and prevention.

Licensure / Certification:
Valid Class E Florida Driver License

Complexity of Duties:
Organization and planning skills are utilized to develop and implement strategies for assisting at-risk youth and their families. Knowledge and experience with prevention strategies are employed to render effective service. Knowledge of the geographical service area, community demographics, ethnic and cultural diversity is required to render effective service. Judgment is required to determine and implement appropriate prevention strategies, to determine and initiate appropriate referrals, and to determine the need for consultation with supervisor
