General Duties

Ability to perform and/or thoroughly review psychosocial assessment of patients admitted to inpatient units. Provide psychosocial input into the treatment planning and treatment team meetings. Assist in coordinating treatment team meetings and treatment planning for each patient assigned including completing clinical documentation necessary for treatment planning processes. Demonstrate ability to provide effective crisis intervention skills as well as facilitating group and individual counseling sessions. Organize and facilitate involvement of families/significant others in the assessment, treatment planning and implementation process and discharge planning including identifying, finding and verifying appropriate
housing for person served. Link patients with community resources as needed. Ensure appropriate documentation related to each patient and is recorded in the service record. Inform case managers, outpatient therapy staff et al. of relevant information pertaining to the patient. Work cooperatively as a team member with other staff, and coordinate treatment planning and discharge planning with the member of acute care and outpatient services as appropriate.
 

Job Requirements

Education:
Master’s degree in social work (MSW) or equivalent.

Experience:
Minimum of one (1) year experience in mental health setting

Licensure / Certification:
None required.

Complexity of Duties:
Ability to coordinate treatment team meetings, treatment planning processes and discharge planning process. Ability to document electronically clinical documentation. Ability to conduct a thorough psychosocial assessment. Requires the ability to work independently, responsibly and with multidisciplinary team. Judgment is employed when making decisions relative to care of patients based on assessment. Discernment is necessary to identify need for collaboration or supervision. Knowledge of Baker Act (Chapter 394, Florida Statute, Mental Health Act) is required.
